All project documentation should be maintained throughout each stage in alignment with agency policies and, where applicable, WaTech requirements and oversight expectations.

 

This task focuses on documenting and monitoring all post‑go‑live defects to understand system stability, user impact, and the effectiveness of ongoing support. Agencies, programs, and projects track defects by severity, business impact, and resolution status. The goal is to identify patterns, prioritize fixes, and ensure timely resolution of issues that affect system performance or user experience. This information supports Stage 9 stabilization assessments and informs decisions about readiness, enhancements, and long‑term support needs. Document all post‑go‑live defects, including severity, business impact, and resolution status.

For Programs:

Maintain a consolidated enterprise defect log. Monitor cross‑agency defect patterns and risks.

For Programs with Agency‑Led Sub‑Projects:

Agencies maintain local defect logs. Enterprise team compiles a master defect resolution report.

For Projects:

Track and resolve project‑specific defects and root causes.
